India’s Aircraft Accident Investigation Bureau (AAIB) has released its preliminary report into the June 12 crash of Air India Flight 171, which killed 260 people, including 19 on the ground. The Boeing 787-8 Dreamliner’s fuel control switches moved from “RUN” to “CUTOFF” within a second, cutting both engines. Cockpit recordings captured one pilot asking the other why the fuel was cut off, with the reply, “I didn’t do it.” The US FAA had earlier issued an advisory on possible fuel switch locking disengagement, but checks were not mandatory. The AAIB has yet to determine the cause or assign blame.
