A preliminary NTSB report has revealed the harrowing details of the UPS Flight 2976 crash in Kentucky on November 4. The McDonnell Douglas MD-11 cargo plane lost one of its three engines during takeoff from Louisville Muhammad Ali International Airport, moments before a massive fire erupted.
