A UK government inquiry has found that a “toxic and chaotic” culture delayed the nation’s COVID-19 response, contributing to approximately 23,000 additional deaths. Chaired by former judge Heather Hallett, the inquiry criticised former Prime Minister Boris Johnson’s indecision, rule-breaking at Downing Street, and the actions of top adviser Dominic Cummings.
